Risk Management Forms

State Insurance Program forms distributed at Directors' Meeting on February 20, 2007 

The Charter School Administrative Office (CSAO) met with Kerry Yoneshige and Julie Ugalde of the Department of Accounting and General Services  - Risk Management Section to review the State Insurance Program (Program).  The Program includes Property, Automobile, Crime, and Liability Insurance.  

If your charter school would like to participate in the State Insurance Program, please complete the attached forms. 

Also attached are instructions for the Data Requirements Form. Be sure to provide the number of school employees for liability insurance coverage. 

If you currently have your own coverage and do not wish to participate, please notify the CSAO.
